Certified Marriage Certificate
This is a little info to help you in obtaining your certified marriage certificate from
Births, Deaths & Marriages after the ceremony.
In regards to the marriage certificate, the one we issue on the day is an official certificate of marriage, it is not a document of identity (such as passport or drivers license) it is evidence of a person’s legal status.
There are 3 documents signed on the wedding day
When signing the documents the bride uses her maiden name
1. Register Book (big red book that we keep)
2. Form 15 Marriage Certificate (given to you on the day)
3. Form 14 Certificate of Marriage, this we send with the NOIM form to the Births, Deaths & Marriages by registered post to be lodged & registered in the state you were married after your wedding.
To obtain the certified marriage certificate from BD&M you can download the Marriage Certificate application form from their website online or call in.This is the document that authorities require these days as proof to change your name on passports etc.

Post to Registry of Births, Deaths and Marriages, PO Box 15188, City East Queensland 4002 or lodge in person at 110 George Street, Brisbane or your local Queensland Magistrates Court or Queensland Government Agent Program (QGAP).
